Application to Access Government Information

You may use this form (pdf 81Kb) to apply for government information. If you are unhappy with DLGH’s decision in relation to your application, you may use the review form to apply to the Department to reconsider that decision.

Application to Access Personal Information

You may use this form (pdf 77Kb) to apply for access to your personal information. If you are unhappy with DLGH’s decision in relation to your application, you may use the review form to apply to the Department to reconsider that decision.

Application to Correct Personal Information

You may use this form (pdf 76Kb) to make an application to correct your personal information. If DLGH refuses to correct the information, you may submit a statement setting out your views to be placed with the information. Use the review process if you are still unhappy with the decision DLGH has made.

Application to Review a Decision

This form (pdf 83Kb) is used when you are not satisfied with the original decision made on your application to view or correct information.

Applicaton to Waiver Fees and Charges

This form (pdf 69Kb) can be used if you consider your circumstances are such that payment of fees and charges would cause financial hardship.

Lodging the Forms

Applications may be submitted by post to: Information Unit DLGH, GPO Box 4621, Darwin NT, 0801 or lodged in person at any Housing Office of the Department. Applications forwarded by mail will need to be accompanied by a photocopy of identification.
